Registration


Start the process:

  1. Complete the below listed enrollment application.

  2. Schedule a tour with our Center Directors.


If PC Tots has open space:

  • Staff will reach out with information about the center location and to schedule a registration meeting.

  • Collect required documentation listed, and bring it to the registration meeting.

  • Complete the Enrollment Checklist.

Required Documentation

You must bring the following to your registration meeting:

  • Verification of child’s age

    • Birth certificate, Medicaid card, baptism certificate, blessing certificate, foster care placement letter, hospital certificate, I-94, or passport

  • Child’s immunization record, as required by Utah Law

    • Required:

      • Polio

      • DTap- Diphtheria, Tetanus, Pertussis (Whooping Cough)

      • MMR- Measles, Mumps, Rubella

    • Children too young for vaccinations are allowed to enroll if when the child comes of age they complete required vaccinations.

  • Proof of income for the previous 12 months

    • Tax forms in the prior year are preferred

    • Also accepted are: W-2 forms, last three check stubs, child support, unemployment, Social Security, or military income

    • Only required if requesting a scholarship

  • Proof of Residency or employment address in Summit County

    • Proof of residence: a recent utility bill, rental agreement, or mortgage statement in parent's name that lists the address

Enrollment Checklist
  • Please complete each step as listed on the right to ensure complete a complete process.

  • If applying for a Brighter Tomorrow Scholarship, you are required to complete the DWS (if eligible) and Upwards applications.

 
 

If PC Tots does NOT have open space, or your child is not born yet:

  • Your child is placed on the Waitlist for all age appropriate classrooms according to the date of application submission.

  • Openings are filled as soon as they become available; if a space is declined, PC Tots will move to the next child on the list.
